Building confidence through healthier choices – Grahame’s story

Before joining the Healthier You: NHS Diabetes Prevention Programme, Grahame did not pay much attention to his eating habits or portion sizes. Like many participants, he was unaware of the impact that small daily choices could have on his long-term health and risk of developing Type 2 diabetes. After learning more about the benefits of making positive lifestyle changes, he began taking a more active role in managing his health.

Our session delivery is targeted to support participants adapt to the lifestyle and diet changes they have made, ensuring they are able to sustain changes and move forwards post programme. Our Health Coaches work closely with participants to develop practical skills around healthy eating, meal planning, portion awareness and increasing physical activity. By helping participants understand how lifestyle choices affect their diabetes risk, they are empowered to make informed decisions that support long-term health and wellbeing.
