Use the Risk Test to start your journey or get referred by your GP. You can take the programme either in group classes or on your own digital journey via an app.
You’ll meet your Health Coach who will remain with you throughout the programme.
You will work through the programme either in your group or on the app and learn a series of behaviour change ideas. These will help you improve your health and lifestyle - and reduce your risk of developing Type 2 diabetes.
You will get lots of support from your Health Coach and other participants in the programme community.

How it works
You will be supported through a series of behaviour change ideas. These will improve your health and lifestyle - and reduce your risk of developing Type 2 diabetes.
If you are taking part in a group class with a Health Adviser there are 13 group sessions in total. All sessions are currently online and on the phone to ensure Covid-19 secure access to the service.
You will work through four key areas of learning: Be Aware, Take Control, Eat Well and Move More.
Sessions are weekly for the first month before moving to fortnightly.
The programme is designed to be informal, interactive and supportive. Everyone will be working together and supporting each other.
As you progress you will start to feel healthier through the changes you make to your own lifestyle. They key part of the programme is ensuring these changes are sustainable - now and in the future.
Your Health Coach will give you all the guidance and support you need.
