About the programme

Your journey

Healthier You: NHS Diabetes Prevention Programme is free to people at risk of Type 2 diabetes.

You may have been identified as at risk by your GP, a health professional or by using the Check Your Risk test.

The programme will help you improve your health and lifestyle - and reduce your risk of developing Type 2 diabetes.

The programme is designed to be informal, interactive and supportive. Everyone will be working together and supporting each other.

As you progress you will start to feel healthier through the changes you make to your own lifestyle. They key part of the programme is ensuring these changes are sustainable - now and in the future.

Face-to-face - How it works

A trained Health Coach will help you make a number of simple and sustainable changes to your lifestyle.

You will be supported through a series of behaviour change ideas in a group.

Over next nine months you will attend 13 sessions with us.

The first four sessions are fortnightly, then monthly from then on.

On your phone or online using our app, plus regular contact from a Health Coach.

Digital - How it works

You will use the Second Nature app to take part in the programme on your phone.

The app will support you through a series of behaviour change ideas with access to a support group and Health Coach. You will receive a recipe book and scales before starting.

Over next nine months you will work your way through the programme to build and maintain healthy habits in the long term.

Getting StartedWeeks 2 to 13Weeks 14 to 40An Adviser from SecondNature will call you. They will talk to you about theapp, the support you willreceive and answer anyquestions you have.You will meet your HealthCoach and support groupthrough the app. Together, you will set realisticgoals and learn to buildbalanced meals, increase youractivity levels, tackle negativethought patterns and setbacks.These will be small, incrementalchanges to a healthier you.You will learn to sustain yournew lifestyle. You will track your progressusing the app and your scales. When you complete theprogramme, you will have a finalconsultation to record your newBMI.

Remote

In online video sessions with a Health Coach if you need additional support with access.

Remote - How it works

If you need support with access or adjusted content, you can take the programme on video call with a trained Health Coach.

They will help you make a number of simple and sustainable changes to your lifestyle.

Over next nine months you will attend 13 sessions with us.

The first four sessions are fortnightly, then monthly from then on.

  • Visually impaired

    Your sessions will be in maximum groups of eight, with support provided by RNIB.

    Sessions will be hosted on Teams.

  • Hearing impaired

    Your sessions will be in maximum groups of eight, with support provided by Clarion.

    Sessions will be hosted on Clarion’s Video Platform with BSL interpreters.

  • Bangladeshi / Pakistani

    Your sessions will feature cultural adaptations, with support provided by Language Line interpreters.

    Sessions will be hosted on Zoom and available in Urdu, Punjabi, Bengali, Pashto and Gujarati.

  • Women with previous gestational diabetes

    Your sessions will be adapted to incorporate appropriate receipes and exercise.

    Sessions will be hosted on Teams.
