Background

Healthier You is a nine-month behaviour change programme that helps supports people at risk of developing Type 2 diabetes.

Participants receive support to change their lifestyle in a friendly and supportive group environment.

Designed by experts, our multi-component intervention is underpinned by a decade of research into community-based diabetes prevention.

Participant journey

On referral, we conduct an individual assessment, including a BMI check and, where appropriate, very brief advice on smoking cessation.

Participants then attend thirteen 90 minute group sessions over 40 weeks. They receive extensive supporting resources, including handouts, trackers and a pedometer.

For those who are not able to attend our group sessions, they can access the service through a digital app or through online remote sessions.

Adults aged 18+

With no Type 2 diabetes diagnosises and who are not pregnant at time of referral

In the past 12 months that indicates prediabetes (HbA1c 42 – 47 mmol/mol; FPG 5.5 – 6.9 mmol/l)

Refer Someone

You can refer someone with an eligible blood test by calling us on 0800 092 1191.

Alternatively, your patient can call us themselves if they have had an eligible blood test.
